Job Description
The RN-EC/NP is an integral member of the renal health care team in the provision of comprehensive health care services. The NP will promote patient wellness, health promotion, and disease prevention for Chronic Kidney Disease (CKD) patients.  Responsibilities of the role include coordinating the plan of care for stable outpatient dialysis and CKD patients with a focus on ensuring that continuity of care is delivered to all patients. Emphasis will be on supporting an environment for patient independence, collaborative self management and health promotion, as well as a liaison to community partners.  Autonomous practice is enhanced by the use of established medical directives and consultation with renal physicians as required.  The RN (EC) will provide care that is related to the dialysis treatment and encourage a shared care model of care with the patient’s general practitioner for non-specific dialysis health issues.   Each hemodialysis patient will be reviewed/assessed a minimum of once a week.
 
The Nurse Practitioner will follow the College of Nurses standards for the Extended Class RN (EC). Nephrology experience is preferred in order to work within the hemodialysis satellite units.  The successful candidate will participate in the ongoing development, implementation and evaluation of the role as program expansion and service growth are operationalized. Contributes to ensuring there is a safe environment for patients, staff and visitors.
 
Job Requirements
•       Graduated from a recognized college or school of nursing
•         Current certificate of competence, Extended Class from the College of Nurses of Ontario in good standing
•         Masters Degree in Nursing preferred
•         BCLS; ACLS preferred
•         CNeph C certification required (within 3 years of hiring)
•         A minimum of  5 years current practice within the NP role
•         Preference given to those candidates with Hemodialysis, peritoneal dialysis and nephrology experience;
•         Effective communication and leadership capabilities;
•         Demonstrated ability to work as a member of a multi-disciplinary team;
•         Demonstrated superior presentation and consultative skills;
•         Demonstrated teaching and mentoring skills;
•         Strong awareness and utilization of clinical best practice and evidence based care
•         Good attendance record.
